Discuss Diabetes

I have attached the first part of the paper. Also I have provided details below for the second paper. It looks like for this section there is no specific quantity of references needed. However, in order to create a budget, implement a plan references are still needed to show where we got the information from.

My quality improvement project: Is to include a diabetic certified dietician who can focus on patients to help improve their blood sugar levels. My first paper shows that lifestyle management (specifically diet) really can make a difference in managing type 2 diabetes. Therefore, the second part of the paper should also have a section that speaks about diabetic dieticians making a difference in type 2 diabetes. Again this part of the paper is where we get to be creative in how we set it up as long as it meets those requirements. The paper should show a PDSA (plan, do, study, act) cycle and swot (strenghts, weaknesses, opportunities, threats) analysis. Analysis diagrams may need to be created to show how the plan was deciding on. I have also attached a different quality improvement project I had for the same california primary health care clinic. Information about the clinic may be useful for this part of the paper. In addition to this I also sent you a sample paper I found online about a quality improvement project when the writer was doing the first part of the paper. The only difference is that this paper is a hypothetical project, we are not really implementing.

Part III:
Your second paper is a disease management program at your CLINICAL SITE /practice of choice. It utilizes all the info that you collected in your review of the literature in the first paper–and its successes are to be measured by the report card tool that you developed in the first paper. You are to outline how you would identify patients, how you would incorporate the standards in their care and your plan for follow up and maintenance of their care. Be sure to address all of the issues you introduced as Standard of Care in the first paper! It is your design, so there is little in the way of a reference list. You need to include a cost analysis of how much it would cost to implement your program and maintain your program (the budget–give it 3 years). Additionally, your business and marketing plan and things like your SWOT would come in to help you with this part.
