a behavioral risk assessment for patients in the ER that have suicidal ideation to prevent them from committing suicide if discharge. Quality initiative to look at what can be done or tools that can be utilized in the ER such assessment of questionnaires to prevent sentinel event from occurring( pt was discharge from hospital with suicidal ideation and went home and committed suicide. This is just the proposal for the capstone on the quality initiative to be done in the ER setting to assess behavioral risk assessment for possible suicidal ideation and how to prevent these pt from missing out on the treatment that they may need . This initiative is to help create a behavioral risk assessment with the research that is avaialbe to prevent averse events from occurring ever again. This will be a project that will be built upon over the next few months where I will be needing your assistance to help create and build upon this project. This is however a 2-3 page proposal on how we will be going about this quality initiative to prevent this suicide from happening again.
